Job Summary

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Lache Health Centre. As a key member of our multidisciplinary team, you will play a vital role in providing and coordinating high-quality, personalised care to our patients.

Main Duties
  • Work closely with clinical and administrative teams to ensure seamless care delivery
  • Provide patient contact and support, including face-to-face and telephone interactions
  • Administer tasks and duties, including patient recall for health checks, QOF, and vaccinations
  • Manage social media and create engaging content
  • Run EMIS reports and create reports as needed
  • Batch messages via ACCURX
  • Provide reception desk support

Person Specification
  • Experience working in healthcare or the voluntary/community sector, supporting vulnerable groups
  • Experience working in multi-professional team environments
  • Experience supporting people, their families, or carers in a related role
  • Desirable: experience using EMIS Web
  • Personal and Knowledge Skills
  • Commitment to continuing personal and professional development
  • Strong organisational skills, including planning, prioritising, time management, and record keeping
  • Good IT skills, including Microsoft Office applications and ability to learn new software packages
  • Ability to actively listen, empathise, and provide personalised support in a non-judgemental way
  • Ability to provide a culturally sensitive service, supporting people from diverse backgrounds and communities
  • Commitment to reducing health inequalities and proactively working to reach diverse communities
  • High level of written and verbal communication skills and ability to communicate effectively with people, their families, carers, and stakeholders
  • Ability to identify risk and assess and manage when working with individuals
  • Strong awareness and understanding of when to refer back to other health professionals/agencies
  • Ability to work collaboratively, liaising with stakeholders as needed for the collective benefit of patients
  • Ability to maintain effective working relationships with colleagues and work flexibly within a team or under own initiative
  • Ability to organise, plan, and prioritise under own initiative and when under pressure, meeting deadlines
