Service Care Solutions is a leading provider of social care services, and we are currently seeking a Mental Health Assistant Coordinator to join our team. In this role, you will work closely with service users, families, and other professionals to deliver person-centred support that makes a real difference in people's lives.

Responsibilities:
  • Develop and implement tailored support plans to meet the specific needs of service users.
  • Collaborate with multidisciplinary teams to provide comprehensive care and support.
  • Maintain accurate records and contribute to the continuous improvement of our services.
Requirements:
  • Significant experience in health or social care, particularly with individuals with mental health conditions.
  • Full UK Driving Licence with unrestricted use of your own vehicle.
  • Qualifications: NVQ/Diploma Level 3 in Health and Social Care (or equivalent).
Salary and Benefits:
  • Competitive hourly rate of £17.12
