Community Health Specialist

We are seeking a highly skilled and compassionate Community Nurse to join our East North East Community Learning Disability Team on a permanent basis. As a key member of our team, you will provide direct clinical care to adults with a learning disability, working to improve their lives and prevent admissions to specialist mental health and general health hospitals.
Key Responsibilities- Provide clinical and operational supervision to junior colleagues, developing intervention toolkits and holding a caseload of care coordination.
- Contribute to service development, projects, and task groups, leveraging your experience and expertise to drive positive change.
- Collaborate with multi-disciplinary and multi-agency teams to promote and advocate for the health needs of adults with learning disabilities.
- Develop and deliver high-quality care, using your experience of leading and delivering care from relevant experience.
- Participate in robust supervision structures, including 1:1s with the Specialist Nurse Practitioner, peer support through weekly MDT meetings, and community nurse meetings.

Requirements- Qualified and registered RNLD/RNMH.
- Experience of working with people with a learning disability and a multi-disciplinary or multi-agency team.
- Must be a driver with access to a vehicle.
