About the Opportunity

The successful candidate will be responsible for conducting comprehensive strength-based assessments, performing thorough housing needs assessments, and ensuring safe and efficient moving and handling practices. You will work closely with clients to develop personalized support plans, promoting their overall well-being and quality of life.

Key Responsibilities

  • Assessment and Planning: Conduct thorough assessments to identify client strengths and needs, informing support plans that cater to individual requirements.
  • Housing and Support Needs Assessment: Perform detailed assessments to identify support requirements, ensuring clients receive tailored assistance and guidance.
  • Moving and Handling Best Practices: Develop and implement safe and efficient moving and handling practices, minimizing risk and promoting client safety.
  • Client Empowerment and Independence: Employ a reablement approach to empower clients, promoting independence and confidence in daily living activities.
