About the Role:
- Provide expert nursing care to patients in their homes, promoting independence and empowering them to manage their health conditions effectively
- Develop and implement personalized care plans tailored to the individual needs of each patient
- Work closely with other healthcare professionals to ensure seamless coordination of care and optimal patient outcomes

What You'll Need:
- Valid NMC registration and proven experience as a Community Nurse or similar role
- Excellent communication and interpersonal skills, with the ability to build trust with patients and their families
- Ability to work autonomously and within a multidisciplinary team, with a strong focus on collaboration and mutual respect
