Community Nurse

We are seeking a highly skilled and dedicated Community Sister to join our Intermediate Care Team at First Community Health and Care. As a Community Sister, you will play a vital role in providing high-quality care and support to patients in their own homes, promoting independence and preventing hospital admissions.
Key Responsibilities:- Assess the health and social care needs of patients and their carers within the community
- Develop and implement personalized care plans to meet the needs of patients
- Work closely with Physiotherapists, Occupational Therapists, and Rehabilitation Assistants to provide comprehensive care
- Communicate effectively with family, carers, and the multidisciplinary team to facilitate rehabilitation and promote independence
- Act as a role model for junior team members and contribute to the promotion of an environment that encourages and supports development of staff using evidence-based nursing practice
- Relevant NMC qualification
- Practice Assessor/Supervisor
- Evidence of Continued Professional Development
- Minimum of 2 years post-graduate experience
- Experience of initiating and participating in clinical audit
