About the Role

We are seeking a highly skilled and dedicated Community Health Nurse to join our Intermediate Care Team at First Community Health And Care. As a key member of our team, you will play a vital role in providing high-quality care and support to patients in their own homes.

Key Responsibilities
  • Assess the health and social care needs of patients and their carers within the community, aiming to prevent hospital admissions and facilitate discharge home.
  • Work closely with Physiotherapists, Occupational Therapists, and Rehabilitation Assistants to provide comprehensive care and support to patients.
  • Act as a role model for junior team members and contribute to the promotion of an environment that encourages and supports the development of staff using evidence-based nursing practice.
  • Utilize a variety of strategies to support patients at home and communicate effectively with family, carers, the multidisciplinary team, voluntary organizations, and other agencies to facilitate rehabilitation and promote independence.
Requirements
  • Relevant NMC qualification
  • Practice Assessor/Supervisor
  • Evidence of Continued Professional Development
  • Minimum of 2 years post-graduate experience
  • Experience of initiating and participating in clinical audit
