About the Role

We are seeking a highly skilled and dedicated Community Health Nurse Specialist to join our team in Bournemouth. This is a fantastic opportunity for an experienced nurse to provide high-quality patient care in a dynamic and supportive environment.

Salary: £29,970 - £36,483 per annum (pro rata for part-time), depending on experience

Job Description:

  • Provide comprehensive nursing care to patients in their own homes, promoting independence and dignity at all times
  • Work closely with multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient
  • Conduct thorough assessments, identify risks, and develop strategies to mitigate them
  • Maintain accurate and up-to-date records, adhering to NHS standards and guidelines
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
Requirements

To be considered for this role, you must:

  • Hold current registration with the NMC
  • Have a valid driving license and access to a car
  • Demonstrate a strong understanding of community-based nursing principles and practices
  • Be able to work flexibly, including evenings and weekends, as required
