Job Description

The successful candidate will work as a member of our high-functioning Intensive Case Management team, supporting adults 19+ experiencing severe substance use concerns. This role involves assessing, treating, and coordinating services for clients who may also have concurrent disorders or co-existing functional impairment. The ideal candidate will possess strong assessment and treatment planning skills, with the ability to provide low-barrier outreach services to vulnerable populations.

This position requires a strong understanding of evidence-based treatment modalities and therapies, as well as excellent communication and interpersonal skills. As a primary clinician, you will work collaboratively with other healthcare professionals to ensure seamless service delivery and optimal patient outcomes.

Required Skills and Qualifications
  • Master's degree in a relevant field (e.g., social work, psychology, counseling)
  • Experience working with individuals struggling with substance use and mental health issues
  • Strong assessment and treatment planning skills
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
