We are seeking an experienced Social Worker to join our dedicated Learning Disabilities team at Tripod Partners. This is an excellent opportunity to make a positive impact on the lives of adults with learning disabilities in our community.
About the Job
As a Care Coordinator for Vulnerable Individuals, you will be responsible for assessing individual needs, developing personalized support plans, and coordinating the delivery of care and support services. Your expertise will enable individuals to live independently, promote inclusion, and enhance overall quality of life.
Key Responsibilities:
- Assess the needs of adults with learning disabilities through comprehensive evaluations and assessments.
- Develop and implement personalized support plans tailored to each individual's requirements.
- Coordinate and monitor the delivery of care and support services, ensuring seamless transitions and effective communication.
- Work closely with families, carers, and other professionals to guarantee the best possible outcomes.
- Promote independence and inclusion for people with learning disabilities, empowering them to reach their full potential.
