Job Description

We are looking for a highly skilled and experienced social worker to join our Adult Community Team as an Adult Community Social Worker.

In this role, you will be responsible for conducting thorough needs assessments, developing and implementing effective support plans, and coordinating services to meet individual needs. You will also be involved in safeguarding vulnerable adults, applying Mental Capacity Act principles, and managing risks associated with the Court of Protection and Deprivation of Liberty Safeguards.

Responsibilities:

  • Conduct Social Care Needs Assessments to identify adult community needs
  • Develop and implement support plans that meet individual needs
  • Coordinate and manage services to meet individual needs
  • Safeguard vulnerable adults and manage risks associated with the Court of Protection and Deprivation of Liberty Safeguards

Requirements:

  • NVQ 2 in Care or equivalent qualification
  • Proven experience as a social worker in adult community services
  • Able to work contracted hours within agreed rota patterns, including evenings and weekends as required
  • A valid car driver's license is essential for travel requirements
