Adult Mental Health Practitioner

EPUT is seeking a motivated and experienced Senior Community Mental Health Practitioner to join our team in Essex. This rewarding role offers the opportunity to provide high-quality care to patients while working as part of a collaborative nursing team.
About the Role
This position involves providing evidence-based, holistic care to service users in line with professional codes and Trust policies. The successful candidate will be responsible for assessing and implementing individualized care plans, ensuring adherence to NICE guidelines and maintaining accurate documentation. Additionally, they will have supervisory responsibilities, overseeing the day-to-day management of lower-banded staff within their professional group and locality.
Main Responsibilities
- Provide and oversee the management and care of service users
- Conduct risk assessments and physical health checks, acting upon findings
- Administer medications, assess side effects, and review medication regimes in line with Trust policy
- Safe transportation and storage of medicines
- Educate and support service users' understanding of mental illness and training packages for staff
Requirements
- Valid UK driving license
- Experience working in community mental health
