About the Role

We are seeking an experienced professional to join our CAD Team as a Adult Care Professional. This critical role within adult care services involves ensuring the safety and wellbeing of service users.

  1. Ratify referrals sent to Bury Adult Care Services to ensure timely and effective intervention.
  2. Contact service users meeting CAD Triage criteria to assess their needs and develop personalized support plans.
  3. Liaise with multi-disciplinary teams, including healthcare professionals, social services, and community groups, to address complex cases.
  4. Locate and engage relevant community resources, such as food banks, support groups, and housing services, to promote holistic wellbeing.
  5. Conduct initial Needs Assessments for STH housing purposes, taking into account individual circumstances and priorities.
  6. Undertake home visits as required to build relationships and provide targeted support.
  7. Recognize and take appropriate action when risk is identified, prioritizing the safety and security of service users.
  8. Support Enquiry Officers in managing calls received, ensuring efficient referral processes and collaborative working.
  9. Process EDT Notifications by conducting further enquiries or signposting to relevant teams, promoting seamless case management.
