About the Role

We are seeking a skilled and compassionate Registered Mental Health Nurse to join our team at Livewell Southwest CIC. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to patients with mental health needs.

Key Responsibilities

  • Assess and monitor patient conditions, identifying areas for improvement and implementing effective care plans.
  • Work collaboratively with the multidisciplinary team to deliver comprehensive care, including medication management and therapy sessions.
  • Develop and maintain strong relationships with patients, families, and carers, ensuring their needs are met and their voices are heard.
  • Participate in regular team meetings and contribute to the development of care pathways and policies.
  • Stay up-to-date with current nursing practices and evidence-based guidelines, applying this knowledge to improve patient outcomes.
