About the Role

We are seeking a highly skilled and dedicated Case Coordinator to join our Education Health and Care Plan (EHCP) service. As a key member of our team, you will play a vital role in supporting children and young people with Special Educational Needs and Disabilities (SEND) to access inclusive and appropriate education.

About the Team

You will be working closely with education providers, developing strong working relationships across education, health, social care, young people, and their families to improve outcomes and ensure access to inclusive and appropriate education for all children and young people with SEND.

About You

We are looking for an individual with excellent communication skills and the ability to work co-productively and creatively with other professionals, children, and young people and their families. You will need to have empathy for the challenges that young people and their families experience, as well as competence in assessing and reviewing the needs of vulnerable children, young people, and their families.
