Job Description

  • We are seeking a Licensed Practical Nurse (LPN) to join our Clinical Care Team. As an LPN, you will work directly with medical providers to deliver high-quality patient care and maintain efficient processes.\
  • You will perform pre-visit planning, including chart review and software utilization to anticipate health maintenance and chronic care needs.\
  • Room patients for provider visits, take vitals, reconcile medications, verify allergies, and prepare refills.\
  • You will also administer vaccines, injections, and PPDs in accordance with standard nursing protocols.\
