The Home First Single Point of Access Team requires a permanent social worker to work on a shift pattern across seven days a week. Our caseload encompasses individuals aged eighteen plus, presenting diverse challenges and opportunities for growth.
We are looking for a candidate who holds a social work degree and is registered with Social Work England. Ideally, you will possess strong interpersonal skills, a proactive approach, and experience working within a busy Social Work Team. This role is open to both experienced workers and Newly Qualified Social Workers, with an ASYE year available.
The successful candidate will be highly motivated, able to undertake comprehensive assessments, support/care planning, and commission services that prioritize person-centered outcomes. You will also provide effective supervision and have experience in social work management. Regular travel throughout North Lincolnshire may be required, and knowledge of adult social care legislation and frameworks is essential.
To excel in this role, you will need to demonstrate problem-solving skills, negotiation expertise, and proficiency in communication, organizational, and IT skills. Flexibility, resilience, and dedication to teamwork are also vital.
