Senior Clinical Fellow in Community Frailty Transformation Specialist

We are seeking a highly skilled and motivated Senior Clinical Fellow to join our team at Guy's and St Thomas' NHS Foundation Trust. The successful candidate will have a strong background in geriatrics, general internal medicine, or primary care, with a passion for advancing their skills in managing complex older people in the community.
Key Responsibilities- Deliver Comprehensive Geriatric Assessment (CGA) and end-of-life care support for older people in the community setting.
- Work collaboratively with multidisciplinary teams to develop and implement innovative solutions to improve patient care and outcomes.
- Contribute to the development of clinical guidelines and protocols to enhance the quality of care provided to older adults.
- Participate in teaching and research activities, including the development of educational programs and the conduct of clinical research studies.
- Full GMC Registration
- Professional Exams - MRCP Part 1 & 2 (GIM/GER/Acute/Palliative care trainees) OR AKT (GP trainees)
- Experience in clinical geriatrics, general internal medicine, or primary care
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment
- Ability to make independent decisions and take calculated risks
