Main Responsibilities:

  • Develop personalized recovery plans in collaboration with patients, their carers, and relevant healthcare professionals.
  • Foster independence by helping patients manage their medication, improve links to the community, and engage in education or support groups.
  • Collaborate with multidisciplinary teams to achieve patients' goals.
  • Support patients in understanding and managing their finances through budgeting work with occupational therapy.
  • Provide signposting to relevant services, such as advocacy, drug and alcohol support, and educational resources.
  • Facilitate group activities on the ward, with training provided.
  • Conduct assessments, including risk and physical health evaluations.
  • Supervise 1-2 rehabilitation workers within the integrated team.

Requirements:

  • Prior experience working with individuals with mental health needs is essential.

Credentials:

  • Nursing degree or equivalent RN(MH) qualification is mandatory.
  • Mentorship qualification or willingness to undergo training is required.
  • Working knowledge/application of the Mental Health Act 1983 is necessary.

Salary and Benefits:

The salary range for this role is £29,970 - £36,483 per annum, gross. This permanent position offers a flexible working pattern and a comprehensive benefits package.
