Clinical Nurse Specialist

We are seeking a highly skilled and experienced Clinical Nurse Specialist to join our team at Sciensus Pharma Services Limited. As a key member of our clinical team, you will play a vital role in delivering high-quality patient care in the comfort of their own homes.
Key Responsibilities- Deliver safe and effective nursing care to patients in their own homes, in line with their individual care plans and needs.
- Conduct thorough holistic assessments of patients to identify their unique needs and develop personalized care plans.
- Administer a range of therapeutic interventions, including IV therapies, as specified in patient care plans.
- Build strong relationships with patients, their families, and other healthcare professionals to ensure seamless care coordination.
- Maintain accurate and contemporaneous clinical records, ensuring they are logged on relevant clinical systems.
- Identify and report any risks or incidents, adhering to contractual requirements and regulatory standards.
- Registered General Nurse (RGN) with current NMC registration.
- Experience in an acute care setting, including some time taking independent clinical decisions and supervising the work of others.
- Strong understanding of therapies and drugs relevant to our service offering, including NHS knowledge relating to the management of patients with treated conditions.
- Excellent communication and interpersonal skills, with the ability to communicate sensitively and effectively with patients, families, and other healthcare professionals.
- Ability to work independently and as part of a team, with a flexible approach to meet changing service demands.
