Position Overview

OneMedical Group is seeking a dedicated Clinical Nurse Specialist/Pharmacist to join our innovative healthcare team. The successful candidate will exhibit proficient clinical judgment and provide exceptional patient care, encompassing assessment, diagnosis, treatment, and follow-up.

Role Responsibilities

The individual in this role will:

  • Serve as the initial point of contact for patients presenting with undiagnosed health issues, managing both urgent and routine consultations.
  • Conduct thorough assessments, diagnose conditions, and develop, implement, and evaluate comprehensive care plans for patients with diverse and complex health needs.
  • Initiate and interpret necessary diagnostic tests and procedures, ensuring accurate and timely results.
  • Facilitate direct admissions to secondary care facilities for patients requiring immediate medical attention.
  • Refer patients to specialized medical services as needed, including expedited referrals when appropriate.
  • Exercise independent clinical decision-making in patient care management.
  • Perform detailed clinical evaluations, considering both physiological and psychological aspects of patient health.
  • Identify and manage treatment strategies for patients at risk of developing chronic conditions.
  • Diagnose and treat both acute and chronic health issues, integrating pharmacological and non-pharmacological interventions into care plans.
  • As an independent prescriber, ensure the safe and effective administration of medications in accordance with current regulations.
  • Monitor and adjust medication regimens based on therapeutic effectiveness and patient-specific needs, adhering to evidence-based practices.
  • Engage with patients to promote adherence to prescribed treatments, providing guidance on medication use, potential side effects, and interactions.

Qualifications

Essential Requirements:

  • Registered with a recognized UK professional body.
  • Possession of a degree-level qualification or equivalent post-registration certification.
  • MSc in Advanced Practice or equivalent, or a commitment to pursue this qualification.
  • Life Support certification.
  • Qualification in Non-Medical Prescribing.

Experience

Essential Experience:

  • Proven experience in managing patients with chronic and complex health conditions.
  • Extensive background in Primary Care settings.
  • Significant post-registration clinical experience.

Skills and Knowledge

  • Advanced clinical practice capabilities.
