Job Summary

The Principal Medical Limited is seeking a highly skilled Pharmacist to join our team as a GP Practice Pharmacist with Independent Prescribing Qualification. This is an exciting opportunity for a motivated and experienced pharmacist to work in a dynamic and supportive environment, utilizing their expertise to optimize patient care and improve health outcomes.

Main Responsibilities
  • Medication Review and Optimisation:
    • To conduct thorough medication reviews and provide personalized recommendations to patients and healthcare professionals.
    • To identify and address medication-related issues, such as polypharmacy, non-adherence, and adverse effects.
    • To develop and implement medication management plans to improve patient outcomes.
  • Medicines Reconciliation:
    • To review and reconcile medications for new patients, ensuring accuracy and consistency.
    • To identify and address discrepancies between medications prescribed by different healthcare providers.
  • Medicines Information/Education:
    • To provide accurate and up-to-date information on medications, including their indications, contraindications, and potential interactions.
    • To educate patients and healthcare professionals on proper medication use, including dosing, administration, and monitoring.
  • Prescribing Systems and Policies:
    • To develop and implement prescribing policies and procedures to ensure safe and effective medication use.
    • To monitor and analyze prescribing data to identify trends and areas for improvement.
  • Chronic Disease Medicines Management:
    • To develop and implement medication management plans for patients with chronic diseases, such as diabetes, hypertension, and asthma.
    • To provide education and support to patients and healthcare professionals on managing chronic diseases.
  • Liaison with Community and Hospital Pharmacies:
    • To establish and maintain relationships with community and hospital pharmacies to ensure seamless medication management.
    • To communicate effectively with pharmacists and other healthcare professionals to resolve medication-related issues.
  • Prescribing:
    • To prescribe medications safely and effectively, taking into account patient-specific factors and medication interactions.
    • To monitor and adjust prescriptions as needed to optimize patient outcomes.
  • Teaching/Training:
    • To provide education and training to healthcare professionals on medication management and prescribing.
    • To develop and implement training programs to improve medication knowledge and skills.
Requirements
  • Essential Qualifications:
    • Mandatory registration with the General Pharmaceutical Council.
    • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
  • Desirable Qualifications:
    • Independent prescriber or working towards/intent of gaining independent prescribing qualification.
    • Membership of the Royal Pharmaceutical Society.
    • A member of or working towards Faculty membership of the Royal Pharmaceutical Society.
  • Experience:
    • Minimum of 2 years post-qualification experience.
    • In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
    • An appreciation of the nature of GPs and general practices.
    • An appreciation of the nature of primary care prescribing, concepts of rational prescribing, and strategies for improving prescribing.
