Pharmacist - Independent Prescriber Specialist

The Principal Medical Limited is seeking a highly skilled Pharmacist to join our clinical team as a GP Practice Pharmacist with Independent Prescribing Qualification. This is an exciting opportunity for a motivated and experienced pharmacist to contribute to the delivery of high-quality patient care and medication management services.
Main Responsibilities- Medication Review and Optimisation:
- Conduct medication reviews and optimisation for patients with complex medication regimens.
- Develop and implement medication management plans to improve patient outcomes.
- Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
- Medicines Reconciliation:
- Review and reconcile medications for new patients to ensure accurate and safe medication management.
- Identify and address medication discrepancies and errors.
- Medicines Information/Education:
- Provide education and training to healthcare professionals on medication management and safety.
- Develop and implement medication information resources for patients and healthcare professionals.
- Prescribing Systems and Policies:
- Develop and implement prescribing policies and procedures to ensure safe and effective medication use.
- Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
- Chronic Disease Medicines Management:
- Develop and implement medication management plans for patients with chronic diseases.
- Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
- Liaison with Community and Hospital Pharmacies:
- Develop and maintain relationships with community and hospital pharmacies to ensure seamless care coordination.
- Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
- Prescribing:
- Undertake prescribing responsibilities as an independent prescriber.
- Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
- Teaching/Training:
- Provide education and training to healthcare professionals on medication management and safety.
- Develop and implement medication information resources for patients and healthcare professionals.
- Essential:
- Mandatory registration with the General Pharmaceutical Council.
- Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
- Desirable:
- Independent prescriber or working towards/intent of gaining independent prescribing qualification.
- Membership of the Royal Pharmaceutical Society.
- A member of or working towards Faculty membership of the Royal Pharmaceutical Society.
