Job Summary

The Principal Medical Limited is seeking a highly skilled Pharmacist to join our clinical team as a GP Practice Pharmacist with Independent Prescribing Qualification. This is an exciting opportunity for a motivated and experienced pharmacist to contribute to the delivery of high-quality patient care and medication management services.

Main Responsibilities
  • Medication Review and Optimisation:
    • Conduct medication reviews and optimisation for patients with complex medication regimens.
    • Develop and implement medication management plans to improve patient outcomes.
    • Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
  • Medicines Reconciliation:
    • Review and reconcile medications for new patients to ensure accurate and safe medication management.
    • Identify and address medication discrepancies and errors.
  • Medicines Information/Education:
    • Provide education and training to healthcare professionals on medication management and safety.
    • Develop and implement medication information resources for patients and healthcare professionals.
  • Prescribing Systems and Policies:
    • Develop and implement prescribing policies and procedures to ensure safe and effective medication use.
    • Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
  • Chronic Disease Medicines Management:
    • Develop and implement medication management plans for patients with chronic diseases.
    • Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
  • Liaison with Community and Hospital Pharmacies:
    • Develop and maintain relationships with community and hospital pharmacies to ensure seamless care coordination.
    • Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
  • Prescribing:
    • Undertake prescribing responsibilities as an independent prescriber.
    • Collaborate with GPs and other healthcare professionals to ensure seamless care coordination.
  • Teaching/Training:
    • Provide education and training to healthcare professionals on medication management and safety.
    • Develop and implement medication information resources for patients and healthcare professionals.
Requirements
  • Essential:
    • Mandatory registration with the General Pharmaceutical Council.
    • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
  • Desirable:
    • Independent prescriber or working towards/intent of gaining independent prescribing qualification.
    • Membership of the Royal Pharmaceutical Society.
    • A member of or working towards Faculty membership of the Royal Pharmaceutical Society.
