About the Role

We are seeking a skilled and enthusiastic Apprentice Pharmacy Technician to join our team at Living Well Partnership. In this role, you will be responsible for providing excellent patient care and working collaboratively with our clinical team to achieve high-quality outcomes.

Key Accountabilities

  1. Perform accurate transcription of medications from hospital discharge summaries and clinical letters.
  2. Independently conduct medication use reviews, including reauthorizing eRD, within your scope of practice.
  3. Ensure accurate and timely maintenance of patient records through medicines reconciliation.
  4. Collaborate with the Practice Based Clinical Pharmacist to deliver medication review services to patients.
  5. Respond to patient and staff inquiries about medications in a professional and timely manner.
  6. Foster partnerships with local pharmacies to optimize patient care.
  7. Provide expert advice on prescribing changes to enhance patient safety, quality, and cost-effectiveness.
  8. Contribute to medication audits and help achieve practice prescribing targets.
  9. Develop and implement strategies for safe medicine sourcing and management.
  10. Engage in multidisciplinary meetings and other forums to promote best practices in prescribing and medicine optimization.
  11. Stay current with ongoing education and training to maintain registration and competence.

What We Offer
