About the Role

We are seeking a dynamic and passionate clinician to join our Perinatal Mental Health Team as a Band 6 Community Mental Health Practitioner. The successful candidate will work with expectant and new mothers with severe and complex mental health problems, providing assessment, interventions, and support in a multi-disciplinary team setting.

Main Responsibilities
  • Manage a complex caseload, including specialist assessment, care planning, and evaluation of individual treatment plans.
  • Participate in the setting of quality standards, auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
  • Administer medication, complying with relevant Trust procedures and NMC Guidance.
  • Assess patient's individual care needs, develop, implement, and evaluate programmes of care.
  • Deliver a range of interventions, including pharmacological and psychologically informed interventions, in accordance with the agreed plan of care and Pathway guidance.
  • Complete and contribute to robust risk assessments and risk management plans, promoting positive risk taking to improve quality of life and maximise occupational performance/independence.
  • Maintain an effective reporting system, observing and reporting verbally and in writing on patient condition.
  • Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.

Person Specification

We are looking for a registered mental health Nurse or Occupational Therapist with post-registration experience, evidence of formal courses and training at diploma/degree level. The ideal candidate will have a BSc degree in nursing/social work/occupational therapy or equivalent level, and qualifications in providing supervision, coaching, and non-medical prescribing. They will also have experience of working with adults experiencing mental health related difficulties, particularly women experiencing perinatal mental health needs, and be able to integrate care delivery across multi-disciplinary/multi agency teams.

The successful candidate will be passionate about working with adults experiencing mental health difficulties, have a flexible attitude, and be able to work across community settings and travel across a geographical area. They will also be able to manage staff/support workers, provide clinical supervision, and participate in Staff Appraisal and Performance Review.
