This is an exciting opportunity for a qualified and experienced Occupational Therapist to join our Integrated Therapy Team at United Lincolnshire Hospitals NHS Trust. You will be working with a multidisciplinary team to provide comprehensive care to older adults, including those with frailty, dementia, acute illness, and long-term conditions.
Responsibilities- Supervise and support other therapy staff within the team.
- Develop and implement individualized care plans to support patients achieving their goals.
- Collaborate with the Multidisciplinary Team to ensure seamless patient transitions throughout the care pathway.
