Job Title: Cluster Team Manager

Aspirations Care Ltd is looking to recruit a Cluster Team Manager to support the delivery of high-quality services to individuals with learning disabilities.

About the Role:

  • Provide leadership and guidance to a team of Support Workers, ensuring that they have the necessary skills and knowledge to deliver exceptional care and support.
  • Develop and maintain accurate and detailed support plans, one-page profiles, and person-centred reviews to ensure that individuals receive tailored support.
  • Liaise with families, advocates, and other professionals to ensure a collaborative approach to support delivery.
  • Conduct regular supervisions and performance appraisals with the team, identifying areas for development and providing coaching and mentoring support.
  • Work closely with the Registered Area Manager to ensure that services are delivered in line with organisational policies and procedures.

About the Service:

The successful candidate will be working with a team of Support Workers to deliver services that provide the best life possible for individuals with learning disabilities.

Key Responsibilities:

  • Lead the support worker team on a day-to-day basis, ensuring that high standards of care and support are maintained.
  • Deliver and demonstrate best outcomes to all individuals supported, ensuring that they are involved in shaping their support plans.
  • Work with the team to ensure that individuals have accurate and detailed support plans, one-page profiles, and person-centred reviews.
  • Co-ordinate all matters relating to medication, ensuring that all procedures are rigorously followed and that support staff are appropriately trained.
  • Support the Registered Area Manager to create a reflective culture of open communication and feedback within the team.
