We are currently seeking an experienced Substance Misuse Nurse to join our team on a full-time basis.
Responsibilities- Develop and implement care plans for patients with complex needs.
- Collaborate with hospital-based specialist teams to ensure seamless transitions between hospital and community settings.
The ideal candidate will possess strong assessment and planning skills, with the ability to work independently and as part of a multidisciplinary team.
Requirements- Qualifications: Qualified Registered Nurse with current PIN and recent post-qualification experience.
- Experience: Prior experience in substance use care, preferably in a hospital setting.
