Job Summary

About the Job:

  • Work collaboratively with multidisciplinary teams to provide holistic care, addressing physical and mental health needs.
  • Develop and implement individualized care plans, incorporating harm reduction strategies, health education, and social support services.
  • Conduct assessments, provide guidance, and facilitate referrals to specialized services as needed.
  • Participate in risk assessments, contributing to risk management plans for individuals engaged in our services.
  • Promote evidence-based practice, staying up-to-date with best practices in addiction care and substance misuse prevention.

Key Skills and Qualifications:

  • Registered nurse or equivalent qualification, with a focus on community health and substance misuse.
  • Minimum 2 years' experience in community-based healthcare settings, preferably in addiction care.
  • Sound knowledge of harm reduction, health promotion, and recovery planning principles.
  • Excellent communication, interpersonal, and problem-solving skills.

Salary and Benefits:

The selected candidate can expect a salary around £36,635 per year, based on experience, along with a range of benefits including flexible working options and opportunities for growth.
