We are seeking a dedicated and compassionate Healthcare Discharge and Support Professional to join our team at Manchester University NHS Foundation Trust. As a key member of our Hospital at Home model, you will play a vital role in reducing length of stay in hospital and promoting patient independence.
About the RoleThis is an exciting opportunity for a motivated and enthusiastic professional to contribute to the development of our service, enhancing the lives of patients with complex needs. You will work closely with multidisciplinary teams, developing and implementing individualized care plans that prioritize patient-centered outcomes.

A competitive salary of ££52,000 - £62,000 per annum recognizes the value of your contributions to our organization.
