We are seeking a compassionate and caring Clinical Healthcare Support Professional to join our team at Manchester University Hospitals NHS Foundation Trust (MFT). As a key member of the Discharge to Assess team, you will play a vital role in providing high-quality patient-centred care to adults in the community in South Manchester.
Your primary responsibility will be to assist patients with personal care needs and provide support under the supervision of Registered Nurses. You will work closely with a multi-disciplinary health & social care team to deliver consistent, high-quality, patient-focused care.
As a Clinical Healthcare Support Professional, you will have an allocated caseload of patients and will be responsible for recording patient observations & changes to clinical conditions. You will also learn how to undertake a range of delegated duties and develop your skills through on-the-job training & assessment.
Previous community experience is preferred, but training will be available. We offer a supportive environment and opportunities for career development through our Support Worker Development Programme for Nursing Assistants.
The estimated salary for this position is £25,000 - £30,000 per annum, depending on experience.
