Job Description

The Christie NHS FT is seeking a highly skilled and dedicated Discharge Nurse Associate to join our team. As a key member of our multidisciplinary team, you will play a vital role in facilitating patient discharges in a timely and safe manner.

Main Responsibilities:

•Communication and Collaboration: Effectively communicate with patients, relatives, carers, and multidisciplinary team members to ensure seamless discharge planning and execution.

•Discharge Planning: Work with the discharge team to develop and implement individualized discharge plans, taking into account patients' unique needs and requirements.

•Coordination and Liaison: Liaise with various healthcare professionals, including doctors, nurses, and therapists, to ensure a smooth transition of care for patients.

•Record Keeping and Documentation: Maintain accurate and up-to-date records of patient care, including discharge plans, progress notes, and other relevant documentation.

Requirements:

•Qualifications: Registered Nursing Associate on the NMC Register with a Foundation Degree and GCSEs Grade C or above in Math's and English Language.

•Experience: Experience of working in teams under appropriate supervision as part of a multi-disciplinary team and providing and receiving complex, sensitive information.

Skills and Knowledge:

•Communication and Interpersonal Skills: Excellent communication and interpersonal skills, with the ability to work effectively with patients, relatives, carers, and healthcare professionals.

•Organizational and Time Management Skills: Ability to prioritize tasks, manage time effectively, and maintain accurate records.

•Knowledge of Discharge Planning: Understanding of the principles and processes of discharge planning, including the development of individualized discharge plans and coordination with multidisciplinary team members.
