Job Title: Advanced Nurse Practitioner

We are seeking a highly skilled and experienced Advanced Nurse Practitioner to join our Integrated Care Coordination Team as a Band 7 HCPC / NMC Registered Clinical Team Lead. This is an exciting opportunity to work in a dynamic and fast-paced environment, providing clinical leadership and management to a team of Clinical Triage staff.

About the Role:
  • Provide clinical leadership and management to a team of Clinical Triage staff, ensuring high-quality, patient-centered care.
  • Develop and implement policies and procedures to ensure safe and effective service delivery.
  • Collaborate with multidisciplinary teams to ensure seamless patient care and promote a culture of quality improvement.
  • Support staff to develop their skills and knowledge, and provide opportunities for career progression.
  • Contribute to the development of the Integrated Care Coordination service, ensuring it meets the needs of patients and stakeholders.
