Paramedic - Primary Care Specialist

We are seeking a highly skilled Paramedic to join our Southampton Central Primary Care Network team. As a key member of our multidisciplinary team, you will provide high-quality, complex care to patients, from initial assessment to diagnosis, treatment, and evaluation.
Main Responsibilities- Care for patients with complex needs, utilizing your expertise and knowledge to deliver enhanced clinical assessments, diagnostics, interventions, and equipment.
- Work collaboratively with our wider health and care team, as well as independently, when conducting home visits in the community.
- Engage patients, families, and carers in co-production of strategies to manage their health and wellbeing.
- Communicate effectively in challenging environments and situations with patients, their families/carers, and the multi-disciplinary team.
- Participate in the clinician's duty rota, including home visits when necessary, and provide access to GPs as required.
- Triage and prioritize patients, including in emergency situations, using a variety of techniques to elicit the history of an event/illness.
- Lead and manage unpredictable and unplanned clinical situations.
We are working with other local NHS Trusts to create a new organization, bringing together community, mental health, and learning disability services from across Hampshire and the Isle of Wight. Our aim is to form the new Trust by the end of 2024, resulting in more consistent care, equitable access to services, and a more sustainable workforce.
Project Fusion is the name for the program to create the new combined NHS Foundation Trust. Our new organization will operate locally to ensure services meet the needs of different communities.
The new Trust will comprise all services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, as well as community, mental health, and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.
Colleagues will transfer to the new organization under Transfer of Undertakings (Protection of Employment) - known as TUPE - on the day the trust is formed.
