Paramedic - Primary Care Specialist

We are seeking a highly skilled Paramedic to join our Southampton Central Primary Care Network team. As a key member of our team, you will provide high-quality, complex care to patients, from initial assessment to diagnosis, treatment, and evaluation.
Main Responsibilities- Care for patients with complex care needs, utilizing your existing knowledge and expertise.
- Conduct face-to-face telephone triage and home visits, working collaboratively with patients, families, and carers.
- Engage patients, families, and carers in co-production of strategies to manage their health and wellbeing.
- Communicate effectively in challenging environments and situations, with patients, families/carers, and the multi-disciplinary team.
- Participate fully in the clinician's duty rota, including home visits and telephone triage.
- Accurately triage and prioritize patients, including in emergency situations, demonstrating the use of various techniques to elicit the history of an event/illness.
- Lead and manage unpredictable and unplanned clinical situations.
We are working with other local NHS Trusts to create a new organization, bringing together community, mental health, and learning disability services from across Hampshire and the Isle of Wight. Our aim is to create a more consistent, equitable, and sustainable healthcare system.
Person Specification- Appropriate professional qualification (nursing or allied health professional) allowing registration with the NMC or HCPC.
- Professional UK registration with the NMC/HCPC.
- Degree-level award.
- Independent non-medical prescribing or pharmacology level 7 profession-relevant qualification.
- Level 7 clinical history taking and physical examination.
- Level 7 diagnostics and decision-making.
- Computer literacy in data entry and communication via IT.
- Teaching and assessing qualification or equivalent mentorship qualification.
- Detailed knowledge of current clinical and professional issues relevant to this field.
- Evidence of developing ability to perform multidisciplinary tasks and work across multi-disciplinary boundaries.
- Evidence of participation in development of clinical practice, supported by advanced theoretical practice knowledge and skills.
- Decision-making at an advanced level, informing clinical reasoning approaches when managing complex situations.
- High-level planning, leadership, and communication skills, including prioritizing competing tasks in a highly complex and dynamic environment.
- Ability to develop positive relationships with internal and external stakeholders.
- Engagement in evaluation, audit, or research activity.
- Confidence in applying and analyzing data.
- Demonstration of required behavior in line with Trust values.
- Specialist clinical skills appropriate to the relevant specialty.
