**About the Service - Housing & Homelessness Services**

A safe home is the solid base from which you build your life. We support people who are experiencing homelessness or at risk of becoming homeless, as well as providing specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women, and young people.

Accommodation to us doesn't just mean a roof over your head and shelter from the elements. It needs to be a welcoming and flourishing environment, offering opportunities for all those we support to feel comfortable and develop.

**About the Job**

Changing Lives provides a Multiple Exclusion Team to undertake outreach within Newcastle up to four times a day, supporting individuals experiencing Rough Sleeping, who are at risk of Homelessness, who are begging, or who are living in temporary accommodation.

The team tries to understand their circumstances, and where appropriate, people are supported into accommodation, other support services, and assistance is provided.

Often the individuals supported by the team are experiencing or have experienced trauma, mental and physical health problems, and addiction. Changing Lives has developed a trauma-informed and therapeutic approach to working with people who are experiencing Rough Sleeping.

We are looking for individuals who are passionate about making a difference in someone's life. The ideal candidate will be dedicated to our Core Values and have the passion and drive to support the people we work with.

**Key Responsibilities**

* Conduct outreach within Newcastle up to four times a day

* Support individuals experiencing Rough Sleeping, who are at risk of Homelessness, who are begging, or who are living in temporary accommodation

* Understand the circumstances of the individuals supported and provide appropriate support and assistance

* Work with the team to develop a trauma-informed and therapeutic approach to working with people who are experiencing Rough Sleeping

* Be dedicated to our Core Values and have the passion and drive to support the people we work with
