As a Band 7 Critical Care Clinical Nurse Specialist, you will have a positive and supportive impact on patients and their loved ones during critical care and recovery. You will work closely with ward areas to support patients and facilitate follow-up clinics post-hospital discharge, following CG83 guidance.
Main ResponsibilitiesThis role supports patients from admission to critical care to the recovery phase. The post holder needs to be a recognised expert in Critical Care and post-Critical Care recovery. Responsibilities include:
- Making early referrals to multi-disciplinary teams
- Facilitating follow-up clinics with clinicians
- Providing specialist advice and support to loved ones and patients along their recovery pathway
- Leading on service improvement projects and working with data and delivery audit reports
