Job Title:

Macmillan Clinical Nurse Specialist (Palliative Care)

Job Summary:

To manage a caseload of patients within specialist palliative care, carrying out holistic assessments and providing individualized patient care.

Main Duties:
  • Provide clinical expertise and specialist advice within palliative and end of life care.
  • Act as a resource and offer ongoing support to ward areas, working alongside staff to ensure the implementation and development of high quality end of life care.
  • Participate in the promotion of patient-centered care, establishing and maintaining a supportive relationship with the patient and their family.
  • Support and enable patients and carers to make informed decisions about their treatment and care options.

Key Result Areas:
  • Provide specialist knowledge and expertise in the delivery of high standards of patient care.
  • Demonstrate appropriate expert practice and specialist advice in the assessment, planning, implementation and evaluation of patient care.
  • Promote and deliver high standards of care at all times which utilizes research based evidence, and is audited and improved accordingly.
  • Plan and deliver education and training for the multidisciplinary team, about palliative and end of life care.
  • Lead and participate in key developments and initiatives relating to specialist palliative and end of life care.
Person Specification:
  • Leadership and motivational skills
  • Ability to demonstrate professional and personal diplomacy
  • Recognition of own limitations
Essential:
  • Experience in caring for patients with palliative care needs and those who are dying
  • At least 3 years qualified B5 or above Registered Nurse
  • Degree in Palliative Care/related degree
  • Excellent communication skills and evidence
Desirable:
  • Teaching experience/Qualification
  • Examples of symptom management experience
  • Evidence of being able to work as an independent practitioner and as part of a team
  • Communication skills course
