Job Description:

The role of a Community Health Specialist at NHS involves providing holistic care to patients accessing primary care services in various environments. The specialist will assess and diagnose patients with acute, non-acute, and chronic medical conditions, making referrals to other healthcare professionals as necessary.

Main Responsibilities:

  • Undertake clinical practice at a specialist level using expert knowledge and clinical skills.
  • Audit patient data to identify areas for improvement and implement changes accordingly.
  • Prioritize patients based on urgency and make decisions about the most effective course of action.
  • Maintain accurate records and ensure compliance with local and national policies.

Required Skills and Qualifications:

  • Independent Prescriber qualification and experience.
  • Clinical supervision training and experience.
  • Full UK driving license.
