Community Healthcare Specialist Job Description

We are seeking an experienced Community Healthcare Specialist to join our team in Southampton. This is a great opportunity for a dedicated professional to make a real difference in the lives of children with complex needs.

About the Role:

  • You will provide face-to-face support to clients and their families, while creating personalized care plans for each person.
  • You will work closely with a core team of carers, training and supporting them with their clinical skills and activities.
  • You will deliver nursing interventions, including medication management.

Requirements:

  • RSCN with an active NMC PIN
  • Up-to-date experience of tracheostomy care and ventilators
  • Suitable for Band 5/6 Nurses with evidence of effective leadership and management of HCAs
  • Full UK driving license essential
