About the Role

As a Reablement Therapist with HCRG Care Group, you will play a vital role in supporting individuals with physical and psychological needs during their rehabilitation journey.

Main Responsibilities

  • Work with a range of physical and psychological conditions, supporting individuals to regain independence in daily living tasks.
  • Deliver rehabilitation and support, promoting independence and restoring functional abilities.
  • Support clinicians with assessment and intervention plans, providing valuable insights and feedback.
  • Carry out basic nursing care tasks, including continence management and prompting medication as necessary.
  • Liaise with colleagues and other agencies to provide a coordinated service, ensuring seamless care for individuals.
