Senior Health Care Support Worker

We are seeking a highly motivated and experienced Senior Health Care Support Worker to join our Community Learning Disability Health Team in Barnsley. The successful candidate will work closely with a multidisciplinary team to provide high-quality care and support to individuals with learning disabilities.
Key Responsibilities:- Deliver planned programmes of care and treatment to designated service users under the direction and supervision of the registered nursing team.
- Support service users to understand their health needs, promote annual health checks, complete hospital passports, and provide health action plans.
- Work closely with carers and families in a range of different care settings and have a caseload under the supervision of a named nurse.
- Contribute to the care planning process, understand the principles underlying care planning, and implement care within well-established policies, procedures, protocols, and guidelines.
- Apply knowledge of longer-term health conditions, provide advice and support, and undertake the physical aspects of care, including moving and handling of service users.
- GCSE A-D or grade 3-9 in Maths and English or equivalent.
- Apprenticeship level 3 or equivalent, such as RQF NVQ level 3 in Health & Social Care or equivalent.
- Care Certificate or required to achieve within 12 weeks of commencement.
- Excellent customer care skills, enthusiastic, and able to work flexibly on a 24hr shift pattern to meet the needs of service users.
- Good IT/keyboard skills, diversity/cultural understanding, and knowledge of responsibilities regarding confidentiality and trust.
