We are seeking a highly skilled SEND Key Worker to join our team at Staffordshire Council on a 3-month contract.

Job Overview:

The successful candidate will be responsible for coordinating the assessment of need and developing education, health & care plans that align with relevant legislation and Code of Practice. This role involves acting as a key worker for adults and children with special educational needs & disability, coordinating support from qualified professionals.

Key Responsibilities:
  • Critical Assessment and Planning: Coordinate the assessment of need and develop education, health & care plans that align with relevant legislation and Code of Practice.
  • Collaborative Teamwork: Act as a key worker for adults and children with special educational needs & disability, coordinating support from qualified professionals, including social work, Educational Psychology, Health, and others.
  • Building Inclusive Relationships: Establish strong relationships with individuals with SEND and their families, respecting diversity and supporting choice and independence to achieve realistic outcomes.
  • Risk Analysis and Management: Analyze written, verbal, and observational information to assess needs, strengths, risks, and potential, identifying and managing risks appropriately while developing tailored strategies.
  • Agencies Collaboration: Work with Families First and independent futures to understand needs and enable individuals to achieve their goals.
  • Innovative Solutions: Explore innovative options with individuals and families to meet agreed outcomes, providing information for informed decisions and ensuring alignment with available services.
  • Support Package Arrangement: Arrange suitable support packages, negotiate costs, allocate funding, and ensure families are informed of financial responsibilities, maintaining transparency and efficiency.
  • Ongoing Case Reviews: Conduct regular case reviews and annual assessments, using evidence to adapt plans and arrangements for achieving outcomes, involving relevant professionals as needed.
  • Record Keeping: Maintain detailed, professional records of assessments, decisions, and performance data, ensuring compliance with SEND EHC policies and safeguarding procedures.
  • Quality Assurance: Engage in quality assurance, performance analysis, reflective supervision, and personal development to enhance SEND EHC services and align with legislation.
Requirements:
  • Qualifications: NVQ3 or equivalent qualification (or a willingness to undertake) and experience in a relevant discipline, such as education, social care, childcare, advice and guidance, or related subject, or equivalent experience.
  • Knowledge/Experience: Significant experience in planning personalized support for individuals with disabilities, promoting independence and tailored progress.
  • Awareness: Awareness of physical, intellectual, emotional, and social development across ages and understanding of family dynamics.
  • Disability Knowledge: In-depth knowledge of disabilities, special educational needs (SEN), and their impact on individuals, families, and society.
  • Teamwork Experience: Proven experience in collaborative teamwork to achieve shared objectives.
  • Empowerment Skills: Skilled in empowering individuals and families to plan for immediate, medium, and long-term outcomes with dynamic options.
  • IT Proficiency: Proficient in using IT for documentation, data management, communication, and reporting.
  • Safeguarding Knowledge: Knowledge of safeguarding, abuse prevention, and relevant legislation, including the Mental Capacity Act, Children Act, and SEN Code of Practice.
  • Partner Organization Understanding: Understanding of the roles of partner organizations and procedures to engage professionals when necessary.

Salary: £40,000 - £45,000 per annum, depending on experience.
