Physiotherapist Community Support Specialist

We are seeking a skilled Physiotherapist to join our D2A service, part of the Intermediate Care team for Somerset NHS Foundation Trust. As a Physiotherapist, you will play a vital role in supporting individuals to regain or maintain their independence in their own homes.
Main ResponsibilitiesKey duties include:
- Conducting therapy assessments, goal setting, and treatment planning
- Assessing risk and promoting independent functioning
- Working collaboratively with provider teams and rehabilitation practitioners

Person SpecificationWe are looking for a Physiotherapist with:
- Active HCPC registration
- Extensive experience in community and acute settings
- Supervision and appraisal experience
