About the Role

We are seeking a skilled Physiotherapist to join our D2A service, part of the Intermediate Care team for Somerset NHS Foundation Trust. As a Physiotherapist, you will play a vital role in supporting individuals to regain or maintain their independence in their own homes.

Key Responsibilities
  • Conduct thorough therapy assessments, including goal setting and treatment planning, to promote independent functioning.
  • Work collaboratively with our provider teams and rehabilitation practitioners to deliver high-quality care.
  • Develop and maintain effective communication and liaison with other Trust teams, Trusts, and external agencies.
  • Use communication tools to gain informed consent from patients, considering barriers such as hearing loss, altered perception, or cognitive difficulties.
Person Specification
  • HCPC registration (or pending due to qualifying).
  • Extensive experience in community and/or acute Physiotherapy.
  • Experience working with frail and elderly populations, providing rehabilitation and supervision.
