Job Summary

We are seeking a skilled Physiotherapist to join our D2A service, part of the Intermediate Care team for Somerset NHS Foundation Trust. As a Physiotherapist, you will play a vital role in supporting individuals to regain or maintain their independence in their own homes.

Main Responsibilities

Key duties include conducting therapy assessments, setting goals, and developing treatment plans. You will work closely with our provider teams and rehabilitation practitioners to ensure effective care delivery. Your expertise will be essential in promoting independent functioning, assessing risk, and enabling individuals to reach their full potential.

Person Specification

We are looking for a candidate with:

  • Extensive experience in community and/or acute Physiotherapy
  • Experience working with frail and elderly populations and providing rehabilitation
  • Supervision and appraisal experience

Additionally, a service development background would be desirable.
