Job Title: Community Paediatrics Nurse Specialist

County Durham and Darlington NHS Foundation Trust is seeking a highly skilled and experienced Community Paediatrics Nurse Specialist to join their team. The successful candidate will be responsible for delivering high-quality care to patients with tuberculosis (TB) in a community setting.

Main Responsibilities:
  • Undertake specialist assessments, diagnoses, and treatment plans for patients with potential or diagnosed TB disease.
  • Act as a point of contact for local community teams and provide education, mentorship, and leadership to junior colleagues and students.
  • Provide direct management for band 5 and administration colleagues, coordinating clinical work and participating in team performance monitoring.
  • Commit to continuous personal development and lifelong learning, attending mandatory training and adhering to trust policies and procedures.
  • Understand own role within the wider multi-disciplinary team and articulate own development needs through clinical supervision, 1:1 sessions, and appraisal preparation.
Clinical Responsibilities:
  • Case manage patients with potential or diagnosed TB disease in a community setting, reporting to the TB specialist nurse lead as appropriate.
  • Holistically assess patient need and develop individualized plans of care that meet the needs of patients and carers.
  • Identify, manage, and review adjustments in optimising TB therapies within agreed service protocols, through non-medical prescribing.
  • Develop, implement, and evaluate patient treatment plans and concordance through clinical checks, tablet counting, and directly observed therapy (DOT).
Educational Responsibilities:
  • Provide and support training for the multidisciplinary team in the management of TB disease.
  • Be a role model, providing mentorship and coaching to the team, and promoting positive well-being support.
  • Regularly attend and participate in training, peer support, and clinical supervision, and promote learning and creation of a learning environment.

The ideal candidate will have post-registration experience of working in healthcare, with a strong background in community nursing and a passion for delivering high-quality care to patients with TB. They will be able to work independently and with minimal supervision, and have excellent communication and organisational skills.

The successful candidate will be based at Bishop Auckland General Hospital, but will be required to travel independently around County Durham and Darlington. They will be part of a dynamic and supportive team, and will have access to ongoing training and development opportunities.
