Job Title: Adult Advanced Practitioner

We are seeking a highly skilled and motivated Adult Advanced Practitioner to join our established team in Acute Medicine. As a key member of our multidisciplinary team, you will provide expert assessment, examination, diagnosis, treatment, and management skills to patients across the adult age spectrum in all areas of acute medicine.

Key Responsibilities:
  • Coordinate the care of acute medicine patients throughout their pathway with the multidisciplinary team and clinical services to ensure effective management.
  • Lead and deliver clinical care across various settings, according to national and local guidelines, ensuring delivery of evidence-based care.
  • Act autonomously to assess, prioritise, and provide expert clinical care, contributing to patients' treatment plans and ensuring quality care.
Delivering a Quality Service:
  • Ensure own and others' clinical practice is research-based.
  • Prioritise, organise, and manage own workload to maintain and promote quality.
  • Deliver care according to local and national guidelines, ensuring delivery of evidence-based care.
  • Initiate and participate in the maintenance of quality governance systems and processes across the organisation.
  • Utilise the audit cycle to evaluate the quality of work and implement improvements where required.
Leading and Developing Practice:
  • Lead a team of trainee and junior Advanced Clinical Practitioners in acute medicine, delivering high-quality and effective care.
  • Monitor the performance of individual trainees and junior Advanced Clinical Practitioners, identifying and implementing actions when standards are not met.
  • Manage specifically identified services or projects, as agreed with the acute medicine group Management team/Clinical Lead/Consultant Nurse.
