We are seeking a highly motivated and enthusiastic Practice Nurse to join our team at Wollaton Park Medical Centre. Main Responsibilities- Provide and maintain a high standard of nursing care to patients, including primary care treatment, preventative care, screening, and education.
- Focus on delivering evidence-based practice for patients with long-term conditions and providing preventative healthcare to our practice population.
- Work effectively with our integrated nursing teams to provide quality nursing care throughout the practice.
- Communicate effectively with patients, carers, and other team members, recognising the needs of individuals with diverse backgrounds and requirements.
- Take an active role in supporting the practice to achieve local and national targets, including the PMS contract, QOF, and Enhanced Services.
- Registered Nurse with sufficient demonstrable experience post-qualification.
- GCSE grade A to C in English and Maths.
Wollaton Park Medical Centre is an 8-partner practice based in the City of Nottingham, serving approximately 10,782 patients.
