About the Job

This is a challenging and rewarding role, requiring a strong background in occupational therapy and experience working with patients with complex needs. The successful candidate will be responsible for developing and implementing individualized treatment plans, using evidence-based practice and client-centred principles to promote optimal health outcomes.

Responsibilities
  • To assess patients' needs and develop treatment plans that meet their goals and objectives.
  • To provide education and support to patients and their families, promoting self-management and independence.
  • To collaborate with multidisciplinary teams to ensure comprehensive patient care.
Qualifications and Experience
  • A degree in Occupational Therapy or related field.
  • Valid HCPC registration and membership of relevant professional organizations.
  • Experience working in a similar role or setting, preferably in a community health clinic.
