About the JobThis is a challenging and rewarding role, requiring a strong background in occupational therapy and experience working with patients with complex needs. The successful candidate will be responsible for developing and implementing individualized treatment plans, using evidence-based practice and client-centred principles to promote optimal health outcomes.
Responsibilities- To assess patients' needs and develop treatment plans that meet their goals and objectives.
- To provide education and support to patients and their families, promoting self-management and independence.
- To collaborate with multidisciplinary teams to ensure comprehensive patient care.
- A degree in Occupational Therapy or related field.
- Valid HCPC registration and membership of relevant professional organizations.
- Experience working in a similar role or setting, preferably in a community health clinic.
- A competitive salary of £45,630 per annum.
- A minimum of 27 days annual leave increasing with length of service.
